If you are wishing to set up a New Patient Appointment with Dr. Bell, we ask that you download and complete the applicable packet:

• New Patient packet for Children and Adolescents
• New Patient packet for ADULTS

After you complete the forms in the packet, please forward the information to us via Fax or email. We will then reach out to you to schedule your appointment.

If you would like us to coordinate care with another provider (for example, primary care physician, pediatrician, therapist etc.), complete this form to authorize release of your psychiatric information:

• Release of Medical Records Form (Please include a photo ID when requesting an electronic release)
• Policy Regarding Release of Information for Patients with Substance Use Concerns
